Getting Ready for ICSI: A Complete Preparation Guide

Written by Ram Prakash, Clinical Embryologist

Do you know that how well you prepare for an ICSI cycle genuinely changes what the laboratory has to work with? At any ICSI clinic in Noida it improves the material we receive, and therefore it improves what we are able to achieve with that material. ICSI gives every single egg the best possible chance of fertilisation, however the quality of the sperm and the eggs which are available still determines your outcome. At Embryologist.co.in, preparation is always a part of the conversation before any cycle begins.

What Should the Male Partner Do Before an ICSI Cycle?

The role which the male partner plays in ICSI preparation gets underestimated very often. Sperm take 64 to 74 days to develop, and what this means is that whatever a man does approximately two months before the cycle is what directly affects the sample which is available on the day of retrieval.

The Most Impactful Steps for the Male Partner

  • Avoid heat exposure because hot baths, saunas and tight underwear all raise your scrotal temperature and suppress spermatogenesis. It is recommended that you switch to looser clothing at least two to three months before the cycle
  • Stop anabolic steroids and testosterone supplements as these shut down your endogenous sperm production and they can cause azoospermia, so the cessation should be happening as early as it possibly can
  • Limit alcohol and quit smoking because both of these are associated with reduced sperm quality along with increased DNA fragmentation
  • Address nutritional gaps where zinc, folate and antioxidant supplementation, meaning vitamin C, E and CoQ10, have some evidence behind them for improving sperm parameters in men who are deficient. Discuss this with your specialist before you start anything
  • Abstain for 2 to 5 days before sample collection because too little abstinence reduces your count while too much of it reduces the motility, and 2 to 5 days is the window which the evidence supports

Moreover, if the male partner requires surgical sperm retrieval, meaning TESE, PESA or Micro-TESE, then that retrieval is typically scheduled so that it coincides with the egg retrieval day, or else the sperm are frozen in advance.

What Should the Female Partner Do Before Starting Stimulation?

Key Steps Before Stimulation Begins

The preparation for the female partner focuses on optimising both the ovarian response and the uterine health:

  • Complete all baseline investigations meaning AMH, antral follicle count, Day 2-3 FSH, LH, oestradiol, thyroid function, and prolactin as well if these have not been done recently
  • Start folic acid at least one month before the cycle, although most specialists will recommend that you begin 2 to 3 months before it
  • Maintain a healthy weight because being underweight and being overweight both affect your stimulation response and your egg quality
  • Avoid intense exercise during stimulation where light activity is perfectly fine, but high-intensity exercise during the stimulation particularly can affect the ovarian response and it risks ovarian torsion as the follicles grow
  • No smoking as smoking is associated with poorer egg quality, with lower fertilisation rates and with worse ICSI outcomes
  • Inform the team of all medications and this includes supplements, herbal remedies and over-the-counter drugs, because some of these interact with the stimulation medications

What Investigations Happen Before an ICSI Cycle Begins?

InvestigationPartnerPurpose
Semen analysis with DNA fragmentationMaleConfirms ICSI is indicated; fragmentation index guides protocol
Infectious disease screen (HIV, Hep B/C)BothRequired before any licensed ART treatment
Karyotype / Y-chromosome microdeletionMaleIndicated in severe male factor or azoospermia
Uterine cavity assessment (saline sonogram or hysteroscopy)FemaleRules out polyps, fibroids, or adhesions before embryo transfer
Endometrial biopsyFemaleSelected cases with prior implantation failure

What Happens at the Clinic During Stimulation?

Once your cycle has started, the female partner attends monitoring scans every 2 to 3 days so that the follicular growth can be tracked and the medication doses adjusted. The male partner is typically asked to produce a fresh semen sample on the morning of the egg retrieval, or earlier than that if a prior arrangement for freezing has already been made. The embryology team then takes over from the point of retrieval onwards, and the ICSI procedure itself takes place within a few hours of that.

Preparation does not guarantee you any specific outcome. However, what it does ensure is that the laboratory team at any good ICSI clinic in Noida is starting out with the best possible biological material, and at Embryologist.co.in that is exactly the starting point which we work from.
